The failure of an aviation mechanic/inspector to adequately inspect the pilot/owner's work during the engine overhaul, which resulted in a loss of engine oil and engine power during cruise flight, and a subsequent ditching. Factors associated with the accident were the inadequate installation of the idler gear shaft setscrew, and the lack of a safety wire on the three-quarter inch hex head (oil) plug in front of the idler gear shaft. Additional factors associated with the accident were the lack of suitable terrain for a forced landing, and ocean water.

An improper flare which resulted in a bounced landing during landing. The pilot's failure to maintain aircraft control and soft terrain were factors which eventually led to the aircraft nosing over.

Jun 22, 2003 Rockvale, TN N555MR Fatal (3) substantial Loss of engine power for undetermined reasons.
Mar 27, 2003 Jay, FL N366X Incident substantial The failure of the pilot to perform a go-around after bouncing following touchdown, resulting in the airplane nosing over during the landing roll.
Jul 08, 2002 Show Low, AZ N9139E Minor substantial the pilot's inadequate compensation for the crosswind conditions and failure to maintain directional control.

the pilot inadvertently flaring the airplane too high above the ground, resulting in a hard landing that sheared a bolt in the tail wheel assembly. This rendered directional control impossible and induced an uncontrolled ground loop. A contributing factor was the crosswind.

Oct 21, 1999 LAUREL, MT N6147M Incident substantial The pilot's inadequate compensation for wind conditions. Factors include a dust devil/whirlwind at the location where the pilot was trying to land.
