Disconnection of the throttle cable at the throttle-to-carburetor adapter plate and the failure of the student pilot to shut the aircraft's engine down once a safe landing was assured. Factors contributing to the accident were the soft terrain and overloading of the nose landing gear assembly.

Feb 28, 1998 WARRENTON, VA N25275 Incident substantial The student pilot's improper flare which resulted in a hard landing.
Feb 20, 1998 NORMAN, OK N5128Q Incident substantial Fuel exhaustion due to the student pilot's failure to refuel. A factor was the lack of suitable terrain for the forced landing.
Feb 18, 1998 AMERICUS, GA N48878 Incident substantial the pilot's improper planning/decision, which led to fuel exhaustion, loss of engine power, and a forced landing on unprepared terrain.
Feb 10, 1998 GEORGETOWN, SC N25952 Minor destroyed

the pilot's decision to attempt a takeoff from an unsuitable area of roadway, following a forced landing after the engine had lost power, due to fuel exhaustion; and his failure to maintain clearance from obstructions, while attempting the takeoff. The obstructions (sign and telephone pole) were related factors.

Feb 08, 1998 ORLANDO, FL N89051 Incident substantial The CFI's improper planning and decision to taxi behind a multi-engine airplane conducting an engine run-up.
Feb 01, 1998 OCHOPEE, FL N94929 Serious (2) substantial

partial loss of engine power due to a fractured number 4 exhaust valve, which resulted in a forced landing; and the lack of sufficient airspeed to maneuver over power lines during an attempt to abort (go around from) an emergency landing, which resulted in an inadvertent stall and subsequent impact with trees. Factors relating to the accident were: the owner/operator's failure to perform the engine manufacture's recommended overhaul at the recommended time, and high obstructions in the emergency landing area.

the pilot's inadequate compensation for wind conditions and failure to properly flare the airplane, which resulted in a hard landing and overload failure of the nose gear. The unfavorable wind conditions were related factors.

Dec 21, 1997 SPRING CITY, PA N22LU Fatal (2) destroyed The pilot's failure to maintain adequate airspeed which resulted in a stall and subsequent in-flight collision with terrain.
