Contributing to the accident was the flight instructor’s delayed remedial action and her failure to ensure that the student had relinquished control of the airplane during the go-around.

The pilots’ inadequate fuel planning and failure to monitor the fuel consumption which resulted in a total loss of engine power due to fuel exhaustion and a subsequent forced landing into wooded terrain. Contributing to the accident was the flight instructor’s inadequate supervision during the instructional flight.

The failure of the propeller blade due to fatigue that originated from an impact pit sustained at an unknown time. Contributing to the accident was the failure of unknown maintenance personnel to detect and repair the impact pit before painting the propeller, which obscured the damage.

The flight instructor’s inadequate supervision and delayed remedial action and the student pilot’s failure to maintain a proper landing flare, which resulted in a nose down impact with the runway.

A partial loss of engine power due to carburetor ice as a result of the pilot’s delayed use of carburetor heat while operating in environmental conditions conducive to serious carburetor icing.
