DURING LANDING, THE MLG BOLT FAILED. INVESTIGATION INDICATES THE CHAMFERED WASHER MS2002C12 WAS INSTALLED INCORRECTLY. BOLT TIME IN SERVICE IS UNKNOWN.
AIRCRAFT ACCIDENT. LT MLG (SPRING) FAILED DURING TAKEOFF ATTEMPT, APPROXIMATELY 6 INCHES ABOVE AXLE. NO EVIDENCE OF PREVIOUS FRACTURE (CRACK) COULD BE DETECTED. LT WING BUCKLED AND AIRCRAFT NOSED OVER. SUBSTANTIAL DAMAGE, NO INJURIES.
IMPULSE COUPLING RIVET BECAME LOOSE IN HUB, ALLOWING INTERFERENCE BETWEEN PAWL AND MAGNETO HOUSING. PAWL HAD WORN THROUGH HOUSING, CRACKING THE MOUNTING FLANGE.
AT 470 HOURS ON REBUILT ENGINE, ALL CYLINDERS WERE REMOVE FOR INTAKE VALVE LEAKAGE. ALL VALVE SEATS WERE EASILY RECONDITIONED, BUT VALVES SHOWED MAJOR IDENTATION AT THE SEAT CONTACT AREA, WITH APPARENT MOVEMENT OF VALVE MATERIAL OUTWARD.
DURING ANNUAL INSPECTION, HORIZONTAL STABILIZER WAS FOUND LOOSE. REAR ATTACH BOLTS HAD LOOSENED. THIS IS SECOND AIRCRAFT. SUBMITTER RECOMMENDS EXTRA CARE DURING INSPECTIONS.
