THE OWNER INSTALLED STC SA01091AT. THIS STC REMOVES THE ELECTRIC COWL FLAP MOTOR, AND INSTALLED A COMPLETELY MECHANICAL SYSTEM. THE OWNER COMPLAINED THAT HE COULD NOT THE CLOSE REAR COWL FLAP IN FLIGHT.
(CAN) FRONT ENGINE WOULD NOT RUN WHEN LT SETTING WAS SELECTED DURING GROUND RUN UP. IT WAS LATER DISCOVERED THAT THE LT MAGNETO NOISE SUPRESSOR WAS ALSO DEFECTIVE, P/N A293.

(CAN) DURING SCHEDULED MAINTENANCE, METAL CONTAMINATION WAS FOUND IN THE OIL FILTER. FURTHER INVESTIGATION REVEALED PARTS OF THE ALTERNATOR DRIVE BUSHINGS IN THE OIL SUMP AND THE ALTERNATOR DRIVE COUPLING AND GEAR DESTROYED. THE ENGINE WAS REMOVED A...

THE LEFT FLAP CONTROL CABLE WAS FOUND BADLY FRAYED AND SEVERED AT THE LEFT INBOARD BELLCRANK. PHOTOS AT THE SCENE SHOW RT FLAPS FULL DOWN AND LOCKED. LT FLAPS ARE NOT EXTENDED. A FULL POWER 'GO AROUND' ATTEMPT RESULTED IN A 'NO CONTROL' SITUATION ...
A CRACK WAS FOUND IN THE REAR OF BLADE SOCKET NR 2 DURING A DYE PENETRANT INSPECTION. THE HUB WAS RED TAGGED AND RETIRED FROM SERVICE. THE PROPELLER HAD BEEN IN SERVICE 3 YEARS SINCE LAST OVERHAUL AND AD 91-15-04 HAD BEEN C/W AT THAT TIME.
