The student pilot's abrupt liftoff resulting in his failure to maintain adequate airspeed followed by a stall. Also causal was the instructor's inadequate supervision.

Dec 04, 2002 Tekoa, WA C-FIIF Serious (1) substantial

The pilot's failure to maintain clearance from the terrain while attempting to fly a non-IFR equipped aircraft solely by reference to its instruments after the aircraft's windshield became coated with freezing rain in cruise flight. A factor was the pilot's improper inflight decision to continue VFR flight in an area where rain showers where taking place while ambient temperatures where indicated to be within one degree of freezing. Additional factors include freezing rain, an iced-over windshield, and tree-covered rising terrain.

Nov 03, 2002 Fairbanks, AK N1933B Incident substantial The pilot's excessive use of brakes during the landing roll, which resulted in the nose over of the airplane.
Jul 18, 2002 Cordova, AK N45796 Incident substantial The pilot's selection of an unsuitable runway for takeoff, which precipitated an in-flight loss of control and subsequent collision with terrain.
Jan 18, 2002 Wahoo, NE N71677 Incident substantial The student, who was a private pilot, used excessive braking during the landing roll.
Dec 07, 2001 Haltom City, TX N1212B Incident substantial The loss of engine power for undetermined reasons. A factor was the lack of suitable terrain for a forced landing.
Nov 25, 2001 Harrisonville, MO N77869 Incident substantial directional control not maintained be the pilot during landing rollout. The ditch was a contributing factor.
Nov 04, 2001 MILLBROOK, NY N1147B Incident substantial pilot's inadvertent activation of the right brake during landing roll-out , which caused the airplane to nose over. A factor was the crosswind.
Aug 04, 2001 Fort Bragg, CA N28882 Minor substantial the pilot's failure to maintain airspeed while making a precautionary landing resulting in an inadvertent stall.
Jul 12, 2001 Resaca, GA N71917 Fatal (2) substantial

The certificated flight instructor's failure to maintain control of the airplane during a turning maneuver, which resulted in an inadvertent stall and subsequent collision with terrain. A factor was the loading of the airplane over maximum gross weight.

Nov 25, 2000 ANTLERS, OK N45537 Minor substantial the loss of engine power resulting from carburetor icing conditions.
Nov 03, 2000 Des Moines, IA N71134 Fatal (1) destroyed

The pilot not maintaining aircraft control during the final turn, and the pilot's improper decision to fly the tight pattern to the closer runway. Factors relating to the accident were the pilot's inadequate distance/altitude in the traffic pattern and the stall.

Sep 30, 2000 HOMOSASSA, FL N2234K Minor substantial

The pilot's inadvertent flight into instrument meteorological conditions and subsequently exhausting his fuel supply. Factors were his failure to obtain a weather briefing, the weather conditions which included clouds and obscuration, the residence and the tree.

Sep 15, 2000 TACOMA, WA N77948 Incident substantial The pilot's inadequate compensation for winds and his failure to maintain directional control. Factors include a gusting crosswind.
Jul 27, 2000 KINGFISHER, OK N71620 Incident destroyed The pilot's failure to maintain aircraft control. A factor was the downdraft weather condition.
Jul 08, 2000 WESTMINSTER, MD N1155B Incident substantial

the delayed corrective action by the flight instructor. Factors were the failure of the private pilot to maintain directional control, his failure to use the brakes, and the lack of brakes on the right side for the flight instructor.

Jun 06, 2000 GREENVILLE, TX N45880 Incident substantial The pilot's failure to maintain directional control during the landing roll, which resulted in an inadvertent ground loop.
Feb 26, 2000 SELMA, CA N1544K Incident substantial The failure and separation of the right landing gear axle during taxi from landing for undetermined reasons.
Dec 26, 1999 GUALALA, CA N45886 Incident substantial The student pilot's failure to attain and maintain an adequate airspeed, which resulted in a stall/mush and a collision with the ground.
Dec 25, 1999 ORLEANS, NE N1363K Fatal (1) substantial the pilot's failure to maintain adequate clearance from the power line. The power line was a factor.
Sep 26, 1999 GRAN MARAIS, MN N65353 Minor substantial the downwind turn, stall, mush, due to the pilot's decision to turn and failure to maintain flying speed.
