Jul 14, 2011 Silverton, CO N1539Q Fatal (2) substantial
The pilot's failure to maintain control of the airplane while operating it at low levels above mountainous terrain and in excess of its maximum allowa…

The pilot's failure to maintain control of the airplane while operating it at low levels above mountainous terrain and in excess of its maximum allowable gross weight, which resulted in an aerodynamic stall. The pilot's failure to maintain control resulted from an acute cardiac event and incapacitation, hypoxia, or the effects of sedating medications or a combination of these factors. Contributing to the accident was the pilot's improper decision to takeoff above gross weight and without oxygen for the flight in mountainous terrain.

Jul 09, 2011 Demopolis, AL N692TT Fatal (7) substantial

The pilot's failure to maintain airplane control during a single-engine approach and his failure to fly an appropriate traffic pattern for a single-engine landing. Contributing to the accident was a total loss of engine power on the right engine due to a fatigue failure of the right engine cam gear.

Jul 04, 2011 Santa Paula, CA N6451X Fatal (2) substantial The pilot’s failure to maintain clearance from a telephone line while flying at a low altitude in dusk lighting conditions.
Jul 01, 2011 Yabucoa, PR N8436Q Fatal (5) substantial

The pilot's inadvertent encounter with a microburst while operating at a low altitude, which resulted in a loss of control from which the pilot could not recover. Contributing to the accident was the pilot’s inadequate preflight planning for the forecasted severe weather conditions.

Jun 28, 2011 Poughkeepsie, NY N61579 Fatal (2) substantial
The pilot’s loss of airplane control due to impairment or incapacitation as a result of coronary artery disease in combination with atrial fibrillatio…

The pilot’s loss of airplane control due to impairment or incapacitation as a result of coronary artery disease in combination with atrial fibrillation. Contributing to the accident was the pilot's failure to report his paroxysmal atrial fibrillation to the Federal Aviation Administration, which would have required follow-up testing that would likely have detected his undiagnosed coronary artery disease.

Jun 27, 2011 Beluga Lake, AK N66286 Fatal (1) substantial The pilot did not maintain adequate airspeed during takeoff, which resulted in an aerodynamic stall.
Jun 26, 2011 Romeoville, IL N732BX Fatal (1) substantial A total loss of engine power due to fuel exhaustion as a result of the pilot’s inadequate fuel planning.
Jun 18, 2011 Armonk, NY N210KW Fatal (4) substantial
The pilot's decision to depart on the flight with a suspected mechanical deficiency and his subsequent decision to fly the final approach at a reduced…

The pilot's decision to depart on the flight with a suspected mechanical deficiency and his subsequent decision to fly the final approach at a reduced power setting. Contributing to the accident was the improper timing of the magneto(s) that resulted in a severe detonation event.

Jun 09, 2011 Vero Beach, FL N48869 Fatal (2) substantial The pilot’s failure to maintain adequate airspeed while maneuvering, which resulted in an aerodynamic stall and subsequent spin.
