The pilot's inadequate flight planning, and lack of suitable terrain for the forced landing. Factor's were the pilot's inadequate aircraft preflight, and the subsequent engine power loss due to fuel exhaustion.

Jun 06, 1999 TIMMONSVILLE, SC N1982Q Minor substantial The loss of engine power due to the improper installation and in-flight separation of an engine cylinder.
May 31, 1999 COLUMBIA, SC N39PC Incident substantial The pilot's failure to maintain directional control during a landing roll.
May 14, 1999 ROANOKE, TX N2343Y Serious (2) substantial The partial loss of engine power for an undetermined reason. A factor was the lack of suitable terrain for the forced landing.
Apr 24, 1999 GREENCASTLE, IN N3270T Minor substantial the pilot operating with known deficiencies in equipment and the fuel cap that disengaged. Factors were the wet grass, and muddy terrain conditions.
Apr 15, 1999 AUBURN, WA N3564F Incident substantial The pilot's improper flare. Contributing factors were gusty, crosswind conditions, and the student's minimal overall experience.
Mar 28, 1999 POTTSTOWN, PA N177AR Incident substantial The failure of maintenance personnel to properly inspect and torque the crankcase bolts.
Mar 17, 1999 CHICO, CA N3406T Minor substantial

The CFI's inadequate supervision and delayed remedial action to rectify his student's bounced landing. Contributing factors were the short runway and obstructing trees.

Mar 06, 1999 LANTANA, FL N2890X Incident substantial the pilot misjudged the flare, resulting in a hard landing, and subsequent impact with the runway.
Jan 01, 1999 MARIETTA, OK N30246 Minor substantial The failure of the pilot-in-command to maintain clearance with terrain. Factors were the short runway and the dark night light condition.
Dec 31, 1998 FORT LAUDERDALE, FL N29451 Incident substantial

The intentional operation of the airplane by the pilot with known deficiencies in equipment (fuel leakage from the carburetor), poor preflight by the pilot for his failure to visually determine that the fuel tanks were full before the initial flight. Factors in the accident were improper overhaul of the carburetor by a facility which would result in an increase in the fuel flow at full rich position, and also, failure of the float hinge arm pin in the carburetor which resulted in flooding at low throttle settings.

Dec 28, 1998 IMMOKALEE, FL N3293T Minor substantial The certified flight instructor's inadequate supervision and his inadequate management of the fuel, which resulted in fuel exhaustion.
Dec 26, 1998 BELLINGHAM, WA N2355Y Incident substantial
Inadequate preflight planning/preparation, and the pilot's failure to maintain altitude. VFR flight into IMC conditions and cloudy weather conditions…

Inadequate preflight planning/preparation, and the pilot's failure to maintain altitude. VFR flight into IMC conditions and cloudy weather conditions were factors.

Sep 27, 1998 BELEN, NM N29572 Minor substantial The pilot inadvertently stalling the airplane.
